Request Equipment from PSMA
PSMA primarily hands out our equipment through referrals from our partner organizations. We can usually hand out smaller pieces of equipment on short notice, such as shower chairs and toilet seat raisers, but for larger equipment like hospital beds and electric wheelchairs, we will require an order through our referral system. We partner with several non-profit healthcare providers and charitable organizations around Tucson who can put in orders with us. If you are unfamiliar with our referral process, please consult our instructions below.

Partner Organization Supply Requests

If you are one of our preexisting partner organizations with our agency password, please fill out the below form to submit a medical supply request for your client/nonprofit.
- Only two orders per month can be placed for the same client/individual - multiple orders from the same organization but for different clients are accepted.
- List all needed items on the form, and we will do our best to fill your order. We cannot add items to the order at the time of pick-up.
- Please wait for a call stating your order is ready before calling us to ask or arriving at our distribution center.
- Please do not submit duplicate order forms for the purpose of rushing it to get completed. PSMA fills supply requests in the order we receive them.
- We ask that clients/partner nonprofits picking up their supplies do not enter our distribution center and "hand-select" items they would like. Much of our inventory is already sectioned out for other orders, has yet to be cleaned/repaired, or is not yet categorized.
- We are very fortunate to be able to re-donate electric wheelchairs to the community - however, please understand we are limited in quantity and with such valuable equipment, we require those in need of such to complete a verified form upon pick-up to ensure they will be used properly!

PMHDC Southwest Medical Aid only accepts orders from our current nonprofit partners, not directly from individuals. If you are in need of medical supplies/equipment, we can connect you to one of our partners that process all supply requests.

Please Note:
PSMA is donation-based, and the supplies we receive vary in condition and age. Our staff and volunteers work hard to repair and clean all donated materials when needed to ensure its future recipients receive durable equipment. Please, check your items before leaving our distribution center to confirm you are satisfied, as we cannot accept "returns" or exchanges later - unless the item is no longer fully functional. We have many who need help as well. Thank you.
